Health Samurai launches Formbox to replace paper, PDFs, and fragmented form workflows in healthcare

Health Samurai has launched Formbox, a form management platform that replaces fragmented, paper-driven form workflows with a unified system that clinical teams control themselves — no engineering required. Formbox, previously available as Aidbox Forms, now operates both as a standalone platform and as an integrated module within the Aidbox FHIR server.

Despite major investments in EHRs and interoperability, healthcare forms remain inconsistent, siloed, and often manual. Hospitals operate hundreds of nearly identical forms across departments. Health IT vendors rebuild the same form engines in every product. Research teams struggle with standardization across sites. Patients enter the same information repeatedly.

Formbox introduces a new approach: a shared, FHIR-native form layer that operates independently from core clinical systems while integrating seamlessly into them.

Rather than treating forms as features, Formbox establishes them as infrastructure.

I skipped a lot of recent conferences to prioritize attending FHIR Camp 2024 in Portugal the week before last. It was well worth the trip! It was great to hear Grahame Grieve give the background story on how FHIR came to be an open source effort. The open source angle is really important and unique in that it will require a lot of meaningful collaboration and perhaps unprecedented transparency (things we're not great at in the US) to advance these standards in the coming years. There is and will be a lot of opportunity for people to develop and offer FHIR expertise.
