Patients complete the same questionnaires through a dedicated app or email. Scores, conditions, and LOINC-coded observations extracted from forms automatically. Doctor's letters generated, results exported to the HIS, patient-reported outcomes scheduled and repeated. Migrated from a self-built form builder to Aidbox and Formbox on FHIR SDC, definition-based extraction, and pre-fill.
