SDC in Production:
One Standard, Every Workflow
How health IT vendors, labs, home care and patient platforms run real clinical workflows on SDC - hands-on implementation stories.
From specification to production
FHIR SDC has become a foundation for real clinical workflows - across providers, labs, home care, and patient platforms. But moving from specification to production takes more than rendering questionnaires.
One of our goals is to bring together the people who are shaping the FHIR SDC standard and the people who are implementing it in real clinical workflows.
Embedding SDC into existing clinical systems and EHR workflows.
Running structured intake, assessments, and care plans on FHIR.
Operationalizing multilingual, offline-capable forms with structured data reuse.

FHIR SDC Beyond Forms: Voice AI Interviews for PROMs
FHIR Questionnaires don't have to be rendered as forms. Learn how Voice AI can conduct natural patient interviews while FHIR SDC drives the conversation, captures structured data, and produces standard QuestionnaireResponses ready for clinical workflows.

Voice-native smart forms: a live GP CCMP + SNOMED demo
How community and open standards let you build complex solutions together - live on stage. As part of his SPARKED work, Liam implemented a complex FHIR Questionnaire for the General Practice Chronic Condition Management Plan (GP CCMP), a huge form with lots of data to enter. Ilya has been building a voice-native SDC form filler; the missing piece was terminology, since AU Core is tightly bound to SNOMED. That's where Dion joined - he built an intelligent SNOMED code search available as an API or MCP. All three pieces come together to fill the GP CCMP form by voice with proper SNOMED coding.

Automating Anaesthesia Documentation with SDC
Mona embeds SDC forms to reduce administrative burden during time-critical anaesthesia workflows. By pre-filling patient history and automatically creating standardized clinical records, clinicians can focus on patient care instead of data entry.

Advancing interoperability in Canadian healthcare
SDC already powers a live referral platform across Quebec. With CarePath, Akinox is building on that foundation to make fragmented health information more usable, helping clinicians access the right information at the right time, reducing hours of administrative work, and bridging today's workflows with a more interoperable future.
